Payment

These Enrollment Terms apply to your use of the AlynMD enrollment and checkout experience and are incorporated into, and governed by, the full AlynMD Services Agreement, available at:

By clicking “I Agree,” checking a related box, submitting payment information, or otherwise completing enrollment, you acknowledge that you have read, understood, and agree to be bound by these Enrollment Terms and the full Services Agreement.

1. Enrollment Only — No Guarantee of Treatment

By completing enrollment, you are requesting a medical evaluation. Submitting information or payment does not guarantee approval for treatment. All medical services, including prescribing decisions, are made solely by licensed physicians exercising independent medical judgment.

A patient–physician relationship is established only if and when a licensed physician approves you for treatment.

2. Role of AlynMD

Alyn MD LLC (doing business as AlynMD) provides a technology platform and administrative services that facilitate enrollment, communication, and access to care. AlynMD is not a healthcare provider and does not practice medicine.

Medical services are provided by independent medical groups and licensed physicians, pharmacies, and laboratories, each of which is solely responsible for the professional services they provide.

3. Accuracy of Information

You agree that all information you provide during enrollment is accurate, complete, and truthful. Inaccurate or incomplete information may result in delays, denial of treatment, or adverse outcomes. AlynMD is not responsible for outcomes resulting from false or incomplete information you provide.

4. Billing and Payment Authorization

Submitting billing information creates a billing profile but does not obligate AlynMD or any medical provider to provide treatment.

Charges apply only as disclosed during checkout and only after required clinical review and approval, where applicable. All services are provided on a cash-pay basis and are not submitted to insurance or government healthcare programs.

5. Telehealth and Communications

If approved, medical services may be provided via telehealth. Telehealth services are not a substitute for in-person care in all situations. You are responsible for monitoring and responding to communications from your care team.

6. No Emergency Care

AlynMD does not provide emergency medical services. If you are experiencing a medical emergency, call 911 or seek immediate in-person care.

7. Governing Terms

These Enrollment Terms supplement, and do not replace, the full AlynMD Services Agreement, which includes important information about dispute resolution, arbitration, privacy practices, limitations of liability, and your legal rights and obligations.

Payment and billing information (as applicable)

Provide and administer the Platform and enrollment experience
Create and manage your account
Communicate with you about enrollment, your account, and support requests
Process payments and prevent fraud
Improve platform performance, reliability, and user experience

We may share information as needed to operate the Platform and provide services, including with:

Service providers (hosting, analytics, communications, payment processing, customer support)
Medical groups, licensed providers, pharmacies, and laboratories involved in your care (if applicable)
Affiliates, successors, or acquirers in connection with a business transaction
Legal and regulatory authorities when required by law, or to protect rights, safety, and security

We may also share aggregated or de-identified information as permitted by law.

4) Health Information and HIPAA

AlynMD takes privacy seriously. Certain health information may be protected under HIPAA and applicable state privacy laws. Where required, AlynMD uses appropriate safeguards and contractual protections to help protect protected health information.

5) Your Choices

Depending on your circumstances and applicable law, you may be able to:

Request access to or correction of certain account information
Control cookies through your browser settings (note: some functionality may be affected)
Opt out of certain marketing communications (administrative messages may still be sent)
